Support Workers provide a vital part in providing the people we support with the care and ability to improve their quality of life.

We are currently looking for a Part Time Outreach Support Worker to join our team in Devizes and Trowbridge. 

Cornerstones supported living service is in West Wiltshire providing support in peoples own homes for individuals with learning disabilities, autism and complex needs. The service supports and enables people we support to lead a full and meaningful life, through support at home and in the community. Everyone has agreed outcome focused plans, in achieving personal goals, supporting social inclusion and promoting community access. Learning, self-development and enjoying life are promoted to achieve everyone feeling part of society.

We currently have an opportunity to support a gentleman in his 40s in his own bungalow who has learning disabilities and can display challenging behaviours. He is physically activities and likes to be up earl an be out and about throughout his day. He enjoys taking trips on buses, shopping, cinema, gardening, recycling and visiting friends and family.

A driver is essential for the position as the gentleman has his own mobility care that he enjoys taking trips out on. 

As a Support Worker worker you will work shifts patterns that are generally 7am-8pm as well as sleep ins from 8pm. 
We have a 39 hour contract but will consider Part Time hours as well.

Support Workers provide a vital part in providing the people we support with the care and ability to improve their quality of life.

Your role as a Support Worker will include:
- Providing 1-1 and group support sessions
- Creating engaging and creative ways to interact with the people you support
- Help with accessing the community and taking the people you support to their appointments.
- Set person-centred goals and agreeing on how to reach them
- Help with personal care when required
- Support the running of the home including the cooking and cleaning.
